We, as representatives of this participating team, in consideration of GSI Sports accepting this team’s application and permitting participation in the Omaha Fall Cup, hereby agree to release, indemnify, and hold harmless GSI Sports, its officials, sponsors, coaches, referees, and representatives from any and all claims arising from injury to any named participant on this team.
We further certify that each registered player is covered by an approved medical insurance plan, as required for youth soccer participation.
We recognize and acknowledge that adverse weather conditions are considered an act of God. We agree to accept all decisions made by tournament officials regarding field playability and the outcomes of competition, with no right to appeal, objection, or compensation of any kind.
