Spartans Futsal Club LLC — Participation Waiver and Release of Liability

By checking the box, I acknowledge and agree to the following:

I attest that the Participant named below is in proper physical condition and has no disability, impairment, or ailment that would prevent him or her from safely engaging in exercise or sports activities. I have been advised to consult a physician regarding the Participant’s involvement in Spartans Futsal Club LLC programs and activities.

I understand that participation in Spartans Futsal Club LLC activities involves risks of injury or death, and I voluntarily assume and acknowledge all such risks. The Participant has my permission to take part in training, competitions, events, activities, and travel sponsored by Spartans Futsal Club LLC.

I understand and agree that this form may be kept by authorized club personnel and that reasonable care will be taken to keep this information confidential. I authorize Spartans Futsal Club LLC personnel to release this information in the event of a medical emergency.

To the best of my knowledge, I certify that the Participant is physically fit to engage in the described activities. If the Participant becomes ill or sustains an injury during such activities, I hereby authorize Spartans Futsal Club LLC personnel to act on my behalf to obtain medical care, and I accept full financial responsibility for such care.

I voluntarily release, discharge, and agree not to sue Spartans Futsal Club LLC or its personnel from any and all liability, claims, demands, or causes of action arising out of or connected with the Participant’s participation in club activities. I further agree to hold harmless and indemnify Spartans Futsal Club LLC and its personnel from any expenses, including attorney’s fees, related to claims for bodily injury or property damage caused by the Participant.

I acknowledge that I have carefully read this waiver and fully understand it is a release of liability. By checking the box below, I am waiving any right to bring legal action against Spartans Futsal Club LLC or its personnel.

Photo, Video, and Social Media Authorization

I grant permission to Spartans Futsal Club LLC to use the Participant’s name, photograph, image, likeness, or appearance in any video recordings, photographs, social media, or other promotional materials related to the club.
I understand that these materials may appear on Spartans Futsal Club LLC’s website, social media accounts, promotional flyers, or other digital or printed media without compensation, and I waive any right to inspect or approve the finished materials.

The COVID-19 pandemic has presented athletics worldwide with many challenges concerning this highly contagious illness. Some severe outcomes have been reported in children, and a child with a mild or even an asymptomatic case of COVID-19 can spread the infection to others who may be far more vulnerable. While it is not possible to eliminate all risks of furthering the spread of COVID-19, Spartans Futsal Club LLC will take necessary precautions and comply with guidelines from the federal, state, and local governments, CDC, OSHA, as well as the NFHS, to reduce the risks to players, coaches, and support staff as knowledge regarding COVID-19 is continually changing, our futsal program reserves the right to adjust and implement precautionary methods as necessary to decrease the risk of exposure for our staff, players, and spectators. Some preventive methods in the futsal program return-to-play plan include but may not be limited to Health screenings before any practice, event, or team meeting, with participation in the activities being limited and/or prohibited where an individual displays positive responses or symptoms. Encourage social distancing and promote healthy hygiene practices such as hand washing, hand sanitizer, etc. •Intensify cleaning, disinfection, and ventilation in all facilities. Educate athletes, coaches, and staff on health and safety protocols. Require all athletes and coaches to provide their water bottles for hydration. The undersigned voluntarily agree to the following waiver and release of liability by signing this form. The undersigned agrees to release and discharge all claims for ourselves, our heirs, and as a parent or legal guardian for the athletes named below against Spartans Futsal Club LLC, its Directors, successors, assigns, officers, agents, employees, and volunteers, and will hold them harmless from all liability or demands for personal injury, psychological injury, sickness, death, or claims resulting from personal injury or property damage, of any nature whatsoever which may be incurred by the athletes or the undersigned relating to or as a result of the athlete participation in athletic programs, events, and activities during the COVID-19 pandemic. The undersigned acknowledges that participating in athletic programs, events, and activities may include possible exposure to an infectious disease, including but not limited to COVID-19. The undersigned further acknowledge that they know the risks associated with COVID-19. Those specific vulnerable individuals may have more significant health risks associated with exposure to COVID-19, including individuals with severe underlying health conditions whose immune systems are compromised. While particular recommendations and personal discipline may reduce the risks associated with participating in athletics during the COVID-19 pandemic, the risk of serious illness, medical complications, and possible death does exist. We(Guardian/parent) knowingly and freely assume all known and unknown risks, even if arising from others' negligence, and take full responsibility for athlete participation in athletics during the COVID-19 pandemic. We (Guardian/parent) willingly agree to comply with the stated recommendations by Spartans Futsal Club LLC to limit the exposure and spread of COVID-19 and other infectious diseases. We   (Guardian/parent) certify that the Athlete is in good physical condition and allow participation in this sport at our own risk.

PARENTS CODE OF CONDUCT

Dear Parents/Guardians,

Before signing this Code of Conduct, please take the time to read and fully understand its contents. Youth futsal is first and foremost for the players. We invite you to support and enjoy watching your children participate in one of the most exciting sports in the world. To preserve the integrity and rhythm of the game, spectator behavior must never interfere with the players, referees, or the free flow of play that defines futsal.

At Spartans Futsal Club LLC, we are proud to have respectful, supportive, and engaged families. We expect parents and guardians to uphold these standards at all times. Sideline coaching, negative comments toward players, or criticism of referees is not permitted.

Parents and guardians are expected to promote a positive environment by showing respect to referees, players, coaches, teammates, and opposing spectators. We encourage applauding effort, skill, and sportsmanship from both teams and supporting coaches and managers regardless of the outcome of the game. Any concerns should be addressed directly with the coach at an appropriate time and location, away from the playing area and players.

For the safety and enjoyment of all participants, spectators must remain at least two yards away from the playing field at all times.

Please be advised that violations of this Code of Conduct may result in disciplinary action, including suspension or removal of your child from Spartans Futsal Club. Foul language, obscene gestures, confrontational behavior, or public disagreement with referee decisions will not be tolerated. Parents and spectators must comply with all requests from game officials and are strictly prohibited from consuming alcoholic beverages at practices or games.

Any parent or spectator who fails to comply with these standards will be asked to leave the playing area, and play will be suspended until compliance is achieved. Spartans Futsal Club reserves the right to suspend or terminate a player’s enrollment due to persistent or extreme sideline misconduct by a parent or guardian. No refunds will be issued in such cases.

Parents and guardians are also responsible for the conduct of their guests and must ensure they are informed of and follow this Code of Conduct.

Thank you for your cooperation and commitment to creating a positive, respectful, and enjoyable environment for all Spartans Futsal participants.

I hereby grant my consent for Spartans Futsal Club to register my player in its name with USYF, U.S. Futsal, United Futsal, Copa Futsal, and RI Futsal tournaments. I acknowledge and understand that a player may only be registered with one futsal club during the 2025–2026 season, which runs from May 28, 2025, through March 1, 2026, and that registration with any additional futsal club during this period is not permitted.

I further understand and agree that should I choose to withdraw from Spartans Futsal Club, no refunds will be issued. If enrolled in a payment plan, I remain fully responsible for completing all scheduled payments regardless of participation status.

**Late Fee Disclaimer: $30 After One Month Overdue**

Dear valued customer,

We want to inform you of our late fee policy regarding payments. Our terms require that all invoices are paid by the due date specified in your agreement or invoice.

In the event that a payment becomes overdue by one month or more, a late fee of $30 will be applied to the outstanding balance. This fee is intended to cover administrative costs associated with managing late payments and is in line with our commitment to ensuring fair and consistent billing practices.

Please note that our aim is not to penalize, but rather to encourage timely payments and maintain a sustainable and efficient billing process. We understand that unforeseen circumstances can sometimes lead to delays in payments, and we are willing to work with our customers to find suitable solutions.

Refund Policy

1. Cancellation by Parent or Guardian (More than Seven Days Before Program Start):

  • If the registration is canceled by the parent or guardian more than seven days before the start of the program, a 50% refund of the balance will be provided.

2. Cancellation by Parent or Guardian (Seven Days or Less Before Program Start):

  • If the registration is canceled by the parent or guardian seven days or less before the start of the program, no refund of the balance will be given.

3. Cancellation by Spartans Futsal Club:

  • In the event that Spartans Futsal Club cancels the program, a 100% refund of the balance will be issued.
