US Youth Futsal (USYF) Waiver Release and Insurance:

I understand that in addition to the league fees, each player must register with United States Youth Futsal (USYF). The $15.00 per player fee and player releases are completed and paid online by the player/parent/guardian (P/P/G).  Players will not be eligible to participate unless payment is received and released authorized. 

The USYF player registration link will be included in the registration confirmation email.

Outdoor player registrations issued from US Club, AYSO, USYSA, State Associations. etc. are not valid for registration with USYF. Once the event begins, there is no refund of the fee.

CRITICAL NOTICE:

Double-check the name of the player and the date of birth (DOB) above to confirm they are input correctly or the USYF registration will not be valid.  Also, if the player has already registered and paid the USYF player registration fee in the current year (8/1/2023– 7/31/2024), to avoid having to pay again, you must enter the player information exactly as it was input the first time the player was input and paid for the current year. Changing the player name or DOB from the previous registration will cause the system to recognize the player as a new player and will charge the fee again.

BC Fire Player Agreement:

I, the player registered with the program (or parent if player is under 18), recognizing the possibility of physical injury associated with soccer. I hereby release, discharge and/or otherwise indemnify BC Fire Soccer Club, its affiliated organizations and sponsors, their employees and associated personnel, including the owners of the fields and facilities utilized for the programs, against any claim by or on behalf of myself as a result of my participation in the programs and/or being transported to or from the same, which transportation I hereby authorize. I also agree that good sportsmanship and a sense of fair competition will govern my conduct on the field.  I understand that players who are guilty of major transgressions (violence, racial abuse, etc.) will be released from the program and no fees will be refunded. Having read and understood the above, all players and parents (if player is under 18) accept this acknowledgement upon registration. If you do not accept these terms, please abstain from registering.

Waiver and Release Agreement:

Recognizing the possibility of physical injury associated with soccer and in consideration for USYA/USSF and its affiliates accepting the registration for its soccer programs and activities (the "Programs") I hereby release, discharge and/or otherwise indemnify USYA/USSF, BC Fire soccer club and its affiliated organizations and sponsors, their employees and associated personnel, including the owners of the fields and facilities utilized for the "Programs", against any claim by or on the behalf of the registrant's participation in the "Programs" and/or being transported to or from the same, which transportation I hereby authorize. My child has received a physical examination by a physician and has been found physically capable of participating in the "Program". Therefore, I assume the financial responsibility for any medical treatment for my child.

BC Fire Code of Conduct

Players:

  • I will remember soccer/futsal is an opportunity to learn and have fun.
  • I will do my best each day, remembering that all players have talents and weaknesses, including me
  • I will treat my coaches, other players & coaches, game officials, other administrators, and fans with respect, especially with regard to religion, race, color, national origin, sex, height, weight, familial status, ability, sexual orientation, and/or other personal attributes. This respect includes communication through the use of social media.  
  • I will play by the rules.
  • I will have a positive attitude and encourage others.
  • I will use good sportsmanship and set an example for others.
  • While traveling, including away tournaments, I will conduct myself in a manner that reflects positively on myself, my family, my team, and BC Fire Soccer Club.
  • I will not possess, consume or distribute alcohol, illegal drugs, tobacco (including electronic products - vaping), marijuana, and/or unauthorized prescription drugs before, during or after any BC Fire Soccer event including games, practices, and team gatherings.
  • I will talk with an adult if I have a concern about guidelines not being followed.

WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19 ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT:

In consideration of being allowed to participate on behalf of BC Fire soccer Club and related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,

2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES or others, and assume full responsibility for my participation; and,

3. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS BC FIRE SOCCER CLUB, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION).

 Refund Policy

Thank you for registering for the BC Fire 2025-26 Winter Training Program. By registering to our (BC Fire) Futsal program, you agree with the payment amount and structure laid out on this registration process.Please review our refund policy:

  • We offer full registration refund if reimbursement is requested 2 weeks (14 days) prior to the start date of the program.
  • We do not offer full nor partial reimbursement for requests within 13 days of the start date of any of our Futsal programs.
  • We do not offer full nor partial reimbursement if a player misses training sessions and/or gets injured during the duration of a program.
