Team Registration

I understand that in addition to the league fees, each player must register with an Organizational Member of U.S. Soccer. Players will not be eligible to participate unless they show a player registration issued from USSSA, US Club, AYSO, USYSA, State Associations. etc. This is an unrestricted tournament and all members of U.S. Soccer are eligible for play. If you require player passes for this event, contact the event director.

I accept that all team fees will be paid by the required date and if not done so, can result in removal from the league.

Nebraska Leagues monitors all weather conditions and will make announcements on any possibility or cancelation of games due to weather conditions. Every effort will be made to reschedule any game(s) canceled due to weather, but due to limited ability in accessing more court time, Nebraska Leagues may not be able to reschedule all games.

I hereby release, discharge and indemnify Nebraska Leagues, Staff and, their agents and employees, all owners and operators of facilities used for events, and all directors, coaches, employees, agents and representatives from all claims, liabilities, damages or causes of actions arising from any connection with my team's participation in all programs.

I hereby grant Nebraska Leagues, their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of the premises used to conduct the event ("Releasees") the absolute right an permission to use photographic portraits, pictures, digital images, and video recording of My Team, or in which My Team may be included in whole or part, or reproductions thereof in color or otherwise for any lawful purpose whatsoever, including but not limited to use in any Nebraska Leagues websites, promotions, or press releases without payment or any other consideration.

I hereby waive any right I may have to inspect and/or approve the finished product or the copy which may be used in connection therewith, wherein My Team’s likeness appears, or the use to which it may be applied.

I hereby release, discharge, and agree to indemnify and hold harmless Nebraska Leagues, their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers from all claims, demands, and causes of action that I or My Team have or may have by reason of this authorization or use of My Team’s photographic portraits, pictures, digital images or videotapes, including any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in the composite form, whether intentional or otherwise, that may occur or be produced in the taking of said images or videotapes, or in processing tending towards the completion of the finished product, including publication on the internet, in brochures, or any other advertisements or promotional materials.

I represent I am at least eighteen (18) years of age and am fully competent to sign this Release.
