Waiver of Liability

Assumption of Risks: I acknowledge that participation in the Activity involves inherent risks, including but not limited to physical injury, illness, and property damage. I voluntarily assume all such risks and understand the nature of the Activity.

Release and Hold Harmless: I hereby release, waive, discharge, and covenant not to sue Twin Cities Sol Futsal Academy, LLC aka TC Sol, Sol of the Cities aka SOTC, Minnesota United FC aka MNUFC and Adrenaline Sports Center, its directors, officers, employees, volunteers, agents, and any affiliated entities (collectively, the "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or any property owned by me, while participating in the Activity.

Indemnification: I agree to indemnify and hold harmless the Released Parties from and against any and all liabilities, claims, demands, actions, costs, expenses, or judgments, including attorney's fees and litigation expenses, arising out of or related to my participation in the Activity.

Medical Treatment Authorization: In the event of injury or illness during the Activity, I authorize [Organization Name] to obtain medical treatment on my behalf if necessary. I understand that I am responsible for all medical expenses incurred.

Acknowledgment and Agreement:

I have read and understand this Waiver in its entirety. I am aware that, by affirming this Waiver, I am giving up substantial legal rights, including the right to sue the Released Parties. I sign this Waiver voluntarily and without any inducement or assurance of any nature.

4 teams may register for each tournament division, and each team is guaranteed three games. Each team may have a maximum of 6 players on the roster. The division may be combined pending the number of registration. 

Photo and Video Capture Consent Form

I, as the parent/guardian of the registrant, hereby grant TC SOL and Sol Of The Cities, Minnesota United FC and Adrenaline Sports Center permission to capture and use photos and videos of my child during their participation in TC Winter SOLStice 2025.

I understand and agree to the following terms:

Purpose: I acknowledge that these images and videos will be used for the following purposes:

Memories: To capture and preserve memorable moments for my child.

Promotion: To showcase talent and enthusiasm in TC SOL and Sol Of The Cities promotional materials.

Education: As coaching and training tools to enhance my child's skills.

Consent is Voluntary: My consent is voluntary, and I have the option to withhold or grant it as I see fit.

Privacy and Safety: TC SOL and Sol Of The Cities is committed to protecting the privacy and safety of participants. Every effort will be made to ensure the responsible use of these visual materials.

Withdrawal of Consent: I may withdraw this consent at any time by notifying support@tcsolfutsal in writing.

By affirmation, I acknowledge that I have read and understood this consent and agree to its terms.

COVID-19 PARTICIPATION WAIVER

This COVID-19 Participation Waiver ("Waiver") is entered into by and between the undersigned Participant and TC Sol Futsal Academy, LLC, collectively referred to herein as the "Parties," in consideration of the Participant's participation in [Program/Event Name] (the "Activity") during the ongoing COVID-19 pandemic.

Assumption of COVID-19 Risks:

I understand and acknowledge that participation in the Activity during the COVID-19 pandemic carries inherent risks, including but not limited to:

Exposure: I may be exposed to COVID-19, a highly contagious and potentially deadly virus.

Transmission: I may transmit COVID-19 to others, including family members, friends, and the general public, even if I am asymptomatic or have mild symptoms.

Protective Measures: I am aware that TC Sol Futsal Academy, LLC has implemented certain preventive measures to reduce the risk of COVID-19 transmission, including sanitization, social distancing, and mask-wearing. However, there is no guarantee that these measures will eliminate all risks.

In consideration of being allowed to participate in the Activity, I hereby release, waive, discharge, and covenant not to sue TC Sol Futsal Academy, LLC, its directors, officers, employees, volunteers, agents, and any affiliated entities (collectively, the "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, illness, or death, 

I agree to indemnify and hold harmless the Released Parties from and against any and all liabilities, claims, demands, actions, costs, expenses, or judgments, including attorney's fees and litigation expenses, arising out of or related to my participation in the Activity, including any claims by third parties.

COVID-19 Guidelines:

I agree to comply with all COVID-19 guidelines and protocols implemented by TC Sol Futsal Academy, LLC including but not limited to mask-wearing, social distancing, hand hygiene, and any additional measures deemed necessary by TC Sol Futsal Academy, LLC during the Activity.

Acknowledgment and Agreement:

I have read and understand this Waiver in its entirety. I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk of exposure or infection. I am aware that, by signing this Waiver, I am giving up substantial legal rights, including the right to sue the Released Parties. I sign this Waiver voluntarily and without any inducement or assurance of any nature.

MINNESOTA UNITED FC
PARTICIPANT WAIVER AND RELEASE

By checking this box, the participant identified below (the “Participant”) or Parent/Guardian on behalf of Participant (for Participants under the age of 18) understands and agrees that participation in the TC Winter SOLStice presented by Minnesota United FC on Monday, February 17, 2025 (the “Activity”) involves a risk of personal injury associated with the Activity and knowingly and freely assumes all risk of personal injury in any way related to, arising from, or associated with Participant’s participation in the Activity, both known and unknown.  Participant, or Participant’s Parent/Guardian on behalf of Participant, represents that Participant is in good health and appropriate physical condition to participate in the Activity.

Waiver and Indemnification.  In consideration of MNUFC accepting the participation of Participant in Activity, Participant and/or Participant’s Parent/Guardian (and all heirs, assignees, representatives and other persons acting on Participant’s behalf) hereby waive, release and discharge Minnesota United Soccer Club, LLC, Major League Soccer, LLC, Soccer United Marketing, LLC, the City of St. Paul, the Metropolitan Council and their subsidiaries, affiliates, officers, directors, owners, employees, agents, independent contractors and successors and assigns (referred to hereinafter as the “MNUFC Parties”), and Sol Of The Cities (the “Activity Sponsor”) from any and all claims and actions, including without limitation claims relating to personal injury, illness (including COVID-19 and other infectious diseases), death, and damage to or theft of property, and the receipt of medical care or treatment for any physical or mental condition arising from or relating to Participant’s participation in the Activity, whether or not caused by the negligence or other acts or omissions of the MNUFC Parties, or the Activity Sponsor.  Participant and/or Participant’s Parent/Guardian (and all heirs, assignees, representatives and other persons acting on Participant’s behalf) further agree to indemnify and hold harmless the MNUFC Parties, and the Activity Sponsor from any claims, liabilities, or expenses (including attorneys’ fees) arising from or relating to Participant’s participation in the Activity.

Publicity Release.  Participant, or Participant’s Parent/Guardian on behalf of Participant, waives any right of publicity and assigns MNUFC Parties and Sol Of The Cities the worldwide license and right to use any photographs, video, or audio containing Participant’s image, likeness, or voice for any purposes, including advertising, broadcasting or promotional material, without further notice and without compensation, in perpetuity.

Acknowledgment and Understanding.  Participant, or Participant’s Parent/Guardian on behalf of Participant, acknowledges and agrees that they have read this Agreement, fully understand its terms and agree that they are freely and voluntarily signing this Agreement.  Participant, or Participant’s Parent/Guardian on behalf of Participant, further understands and agrees that Participant and/or Parent/Guardian for themselves and on behalf of Participant: (a) are consenting to Participant’s participation in the Activity; (b) understand that all risks in any way relating to the Participant’s participation in the Activity are expressly assumed; (c) are giving up rights, including the right to compensation for injury resulting from Participant’s participation in the Activity; and (d) are providing a complete and unconditional advance waiver and release of all liability to the greatest extent allowed by law.
