I approve of my daughter's/son's attendance at the 2023 Elite 120 Combine and certify that she/he is in good health and able to participate in program activities. I hereby authorize the directors of the 2023 Elite 120 Combine and their employees and/or volunteers to act for me according to their best judgment in any emergency requiring medical attention, including treatment by a physician. I hereby release 2022 Elite 120 Combine/GSI Sports and it's staff from responsibility or liability for injury while participating in the camp.

GSI Sports thank you for registering your child for the 2025 Elite 120 - Combine.

If you have paid by credit card please note that HTG Sports is the billing body which will appear on your credit card statement--please do not cancel this transaction as we will be billed a $25 cancellation fee which we will pass along to you.

Further camp details and check-in information will be forwarded to you prior to the camp. Drop off and pick up times will be clearly identified at that point and any forced weather updates or changes will be communicated.
