I understand that in addition to the Futsal ID Trial player registration fee ( $240).

Each player must register with United States Youth Futsal (USYF) yearly. The $13.50 USYF player fee and player releases are completed and paid online by the player/parent/guardian (P/P/G). Players will not be eligible to participate in any USYF event, unless payment is received and released authorized.  Outdoor player registrations issued from US Club, AYSO, USYSA, State Associations. etc. are not valid for registration with USYF.

If you have already paid the USYF Player registration fee for the year 2023-24 then you DO NOT need to pay and register again. 

Double-check the name of the player and the date of birth (DOB) above to confirm they are input correctly or the USYF registration will not be valid. 

I understand,

If the event gets cancelled due to Covid-19 circumstances or any circumstances out of USYF control we will issue a refund a total of $185.

I understand that I can cancel the registration before May 9th 2024 and receive a refund minus a $35 administrative fee.  After the registration deadline, no refunds will be given.

Thank you for registering for the NC State Futsal ID Trials.  You may cancel your registration before the May 9th, 2024 and receive a refund of your fees minus a $35 administrative fee.  After the registration deadline, no refunds will be given.
